Immunization and Dental Print

Dental exams are required for all Kindergarten, 2nd and 6th grade students. These exams are due by May 15th of the current school year.
All K students are required by law to have an eye exam by a qualified eye doctor. The eye exam must take place within a year prior to entering K. Proof of this exam is required by the first day of school.

Physical exams are required for students entering Kindergarten, 6th and 9th grades. (Sports physicals are not accepted). Physicals must be turned in by the first day of school no appointments will be accepted.

  • Within one year prior to August of the current school year, upon the first day of attendance, students shall show proof of having undergone a health examination by a medical doctor, advanced practice nurse or physician assistant. Students must also show proof of having received the immunizations against preventable communicable diseases as required by the rules and regulations of the Illinois Department of Public Health.
  • If a student does not comply with the requirements of this policy such student will be excluded from school until proof of having received the required health examination and immunizations is presented to the appropriate school nurse.
  • New students to the district are allowed thirty (30) days following registration to comply with the health examination and immunization regulations.
  • During a student's exclusion from school for non-compliance with this policy, the student's parents shall be considered in violation of section 27-8 of The School Code of Illinois and subject to penalty imposed by section 27-8 of The School Code of Illinois.
  • The Lee County Health Department provides immunizations by appointment only. The fee is $8.00 if the family income is under $60,000. Any family whose income exceeds $60,000 will be charged $10.00 per shot. Contact the Health Department at 815-284-3371 for more information.
